I seem to be confused on the prologue and chapter 1 as to when this challenge actually starts. So does the prologue count for the alarm/kill nobody trophy?

Total Awards: 7Alarm? No it doesn't.
Killing? Yes it does. I had the same question. It's from the very start to the finish of the game. You can't kill anyone. This especially includes the prolougue.
You can - of course - kill the bosses.

Total Awards: 7There are alarms, which is the point. It's pretty stupid to be given a trophy for not setting off alarms when the alarms turning on is actually part of the story and can't be changed under any circumstances.

Follow-up question: Just started the game and going for Stealth. In my lower left corner of the HUD I often see a status like "suspicious" or even "hostile" (the first security camera / turret combination in the manufacturing plant for example). Does this constitute an alarm? I want to get across the way to get the hacking nuke, but I can't seem to get across without raising the level to "hostile". I tried to throw crates into the opening and sneak through behind them, but I'll still be seen...

Total Awards: 7It depends where actually.
I can answer part of your question; neither suspicious, nor alarmed from turrets, cameras or guards will trigger an alarm. With cameras, however it will if it turns hostile. Turrets don't count even if they're hostile. As long as no alarm panel is reached by guards you'll be safe.
