I swear I saw on the title screen that there is some item unlocked for having a FFXIII save file, but I've been unable to find this item. It said something about going to your inventory to find it there...did anyone else get this item?

The only thing I've noticed that it unlocked is a pretty decent XMB theme. You download it from the Bonus section in the main menu. The message said I'd earn / notice other bonuses throughout the game, but nothing yet.

There are two things you get from having a FFXIII save file I noticed.
One is a new THEME available, that is of Lightning taken from the open sequence of FFXII-2
And the second is a item in your inventory called the "Eternal Crystal".
From what I can gather from my brief perusal through the game guide, on completion of the task that gets you the "Defragmented Trophy- collect all 160 fragments , [which opens the "Serah" Theme], having this additional item the game treats it as fragment 161 and apparently gives you one extra Paradox Ending, that otherwise is not available.

It's very vague on the additional content that is available to those who bought the + editions.
I know that when you download the extras code for the bonuses you get the availability of getting OMEGA to fight along side you.
To face OMEGA you have to go to the History Crux in Chapt 2, select " Col+som-??? AF-" [I might not have written that down quite right when the info appeared briefly at the PStore ], but it's pretty close.

As for getting the Muramasa for Noel [another bonus code], from what I can gather it should appear as a hidden pick up [probably requiring KUPO to open it]

I got some kind of crystal, its in the key items section...so I think its useless....but I got Azrael for Serah from the XBL marketplace
