Personally I found RE5 professional pretty difficult, took me a few levels to get to grips with how aggressive the majinimotherfuckers were...

easy once you'd unlocked the infinite rocket launcher for finishing the game in under 3 hours

Star Wars: The Force Unleashed, on Sith Master difficulty is brutally hard and will kick your ass over and over, but its doable.

Streets of Rage 2, on Mania difficulty, nigh on impossible, i'm a total master at the game and even i can't do Mania.

I think i got 3 that comes to mind. Killzone2 on elite, COD Classic on vet and MOH Frontline on the hardest dif.

Resident 5 on Professional without infinite/upgraded weapons.
Left 4 Dead on the hardest difficulty, without proper co-op partners it's hard (damn you strategically placed Tanks).
Dead Space 2 on Hardcore. Probably a Trophy that will never let me get the Platinum.
VVVVVV with that mode that you can only die once.
Diablo II on Hell difficulty and Hardcore mode enabled. Granted, being leveled up makes it playable, but if you die, your character gets deleted. It happened to me once, and it sucked. Sucked big time.

Same here, but as for games I did finish/platinum I'm gonna say the hardest for me was Black Ops. Damn Tryarch and their infinite spawn points! I know that's pretty sad compared to some of you people.

Catherine's Hard Mode - Try beating Hard mode without a guide plus some level on the game is really cheap even with a guide..

I found insanity farily easy in Mass Effect 2

Anyhow since the OP said no NG games which even though they would be almost the top choice
so I guess next would be Killzone 2 Elite mode was rough. WipEout HD Elite was also hard as well doing the races in phantom class, really fast it was.
The one game that beats all of them, even NG games is Battletoads, it doesn't have or need to have hard mode lol.

I agree Battletoads was crazy hard! Also GoW 1 god mode was pretty evil from my own experience.
